Fanlistings on trouble for "codes"?

I looked at one of the recent troubles updates, and couldn't figure out what "codes" meant. I went and checked some of the sites to see if there weren't any codes, but that didn't seem to be a problem. Exactly what does this mean? I've never seen it before, so it must be a new thing.

Any fanlisting placed on troubles because of codes, have codes that are ambiguous, ie the code buttons imply that the fanlisting is for something other than what it is.  Examples: A relationship fanlisting that has codes with only one person's picture and only the word fan or a series fanlisting that has a character's name instead of the series name (in both cases they could be confused with a character's fanlisting instead of the fanlisting for the relationship or series).
